Do you need motivation for the nutrition or the fitness piece? If you've already developed an "attitude of gratitude" for your nourishment and God's provision, then you sound pretty motivated in that area. The only other thing I would encourage you to do is to eat as many foods as close to the way God created them as possible.
As for fitness (which is really my struggle and not necessarily yours), as long as I put my body in motion---for at least five minutes--I'm good. I usually aim for a minimum of 10 minutes, and it's surprising how after I've warmed up and gotten into it, I can usually squeeze out another 10, then another 10, etc. But on the days when I'm not up to it, ANY movement is better than just sitting on the sofa watching TV...and I even try to do chair exercises while I'm sitting.
