I remain stunned that the biggest change I've made is to track my food and become more aware of my calories. I know that other elements have a metabolic impact, and I am also doing low-carb, so I think that makes a difference, but the biggest change has simply been waking up to the amount of and quality of food I had been eating.
But the ability to change my eating habits is based on the fact that I'm no longer depressed and no longer using food as a buffer for my raw emotions or a source of pleasure in a painful existence. The emotional healing had to come first for me. (YMMV)
