How do you set up a Prayer Journal?

I have been googling how to set up a prayer journal. I have one a friend gave me to put my own personal thoughts and feelings, but I have a notebook binder I'm setting up for who/what to pray for.

Set up a separate page for each person? Prayer request on one side of page and answer on other side with line drawn separating.

Do you do all prayers everyday? I've read where people will pray for certain people (family, friends, church friends, ministries) on different days.

I also have a Bible plan and Read the Daily Bread and In Touch as part of my devotions for the day. I like to try to get it all in before DH gets up and the quiet is gone.

I did set up name with a post-it note below and prayer request on the note, saw that somewhere.
